- Appointments required: Medical Marijuana Consultants are available by appointment only.
- Already have your authorization form? Call or email your closest AGT location to schedule.
- Bring to your appointment:
- Original Medical Marijuana Authorization Form (tamper-resistant)
- Valid Photo ID
- At your appointment, we will: Register you in the state database, take your photo, and issue your Medical Recognition Card.
Tax note: Recognition Cards allow sales tax exemption, and qualifying DOH-compliant products may also be exempt from the 37% excise tax.
To ensure medical patients receive proper attention with minimal wait times, our Medical Marijuana Consultants are available by appointment only.
